Admin not having WP backend menu items

We have the pro edition, and we have had a problem when about 20% of the time that an admin logs in, they are not logged in by WP and only get the few menu items on the backend Dashboard that a subscriber would get. They have to logout and log back in 3 or 4 times before the Admin log in “takes” and they see the full Admin backend dashboard menu items.

This happens even if their only capability is “Admin.” What do you think causes this behavior?
